La Vergine col Bambino appare in alto, fra nuvole ed angeli nuvole, ai Santi Vescovi Nicola di Bari e Gennaro. Il Bambino, in piedi sul grembo materno, tocca il suo seno. Il latte della Madonna ne simbolizza la pietà e la benedizione. San Nicola, inginocchiato a sinistra, guarda la Madonna e il Bambino in adorazione. San Gennaro, a destra, mostra la miracolosa ampolla del suo sangue. La fonte per il titolo popolare di questo quadro, la Madonna della Purità, è sconosciuta. La Madonna della Purità fu eseguita in collaborazione da Mattia Preti e suo fratello Gregorio. Il contributo di Mattia fu limitato alla figura di San Nicola di Bari che risalta per la sua monumentalità e il chiaroscuro caravaggesco. Gregorio, il membro anziano della società in quel periodo, dipinse tutte le altre figure. Gregorio tendeva a porre le figure in pose statiche e dritte, come il San Gennaro dimostra.

The Virgin and Child are represented on the top of the work, between clouds and angels, and the Bishops Saints Nicholas of Bari and S Gennaro. The Child, standing on the womb, touches her breast. The Milk of the Virgin symbolizes the mercy and the blessing. San Nicholas is kneeling on the left looking at the Virgin and Child in adoration.San Gennaro,on the right, shows the miraculous cruet of his blood. The source for the title of this popular framework, Our Lady of Purity, is unknown. Our Lady of Purity was performed in collaboration by Mattia Preti and his brother Gregory. The contribution of Matthias was limited to the figure of St. Nicholas of Bari, while, Gregory, the senior member of the society in that period, he painted all the others figures. Gregory sought to lay the figure in static pose and straight, like the San Gennaro shows.
